4th Grade Field Trip Form
Thank you for choosing Mission San Luis Rey for your Mission field trip. To request a date, please complete and submit this form. You will be contacted by our museum staff to confirm your requested date. Please plan for your field trip at least 6 weeks in advance.
Starting August 1, 2026, admission is $5 per student. The cost for teachers and chaperones is also $5 per person. Discounts are available for Title I schools. Please let us know your Title I status when submitting your application, under the section “notes to staff”.
All tours begin at 10:00 am. Check-in is at 9:45. Please check in at the Museum desk. We recommend to plan between 2.5 to 3 hours for the docent tour, museum visit and lunch. Payment is due on the day of the tour at check-in. We accept all forms of payment
Please make sure to fill out all fields marked as (Required) as they are mandatory. Upon successful submission, you will receive on-page confirmation message, knowing your submission was received by our staff.
